“Herxheimer reactions”. The name sounds like a sneeze, but in reality, a Herx is uncomfortable and if not handled properly, it can slow down your overall recovery process.

Other terms for this type of response are die-off symptoms, healing response, healing crisis…all sorts of names for this fun (not!) experience.

In this post, we’re going to talk about:

The backbone of the H*E*A*L* Program : Recovering from Long Covid and ME/CFS course. is you learning how to treat and eliminate the underlying root cause:  Hidden Infections.  These infections are things like bacteria, yeast, and parasites.  When these little suckers die, they release toxins.  Let’s take a look…

Why’s It Called a Herxheimer Reaction?

At the end of the 19th century, European dermatologists Adolf Jarisch and Karl Herxheimer were making waves in the world of medicine by treating syphilis with mercury (which acted as an antibiotic).  They observed that the skin lesions got worse before they began to get better and eventually heal.

This concept of “getting worse before it gets better” is seen across the board in treating microbes, so the name caught on.  It’s technically called a Jarisch-Herxheimer reaction but hey, I don’t think either will mind now if we just call it a Herx for the sake of time (and for the sake of my typing fingers;).

Even though the term Herx originally only referred to spirochetal infections (like Lyme, borrelia, etc), it’s now applied in a broader sense to the concept of symptom flares (or even new symptoms sometimes) after starting anti-microbial treatments.

“Whyyyyyyy…😫???” (Why Does a Herx Happen)

I don’t know about you, but when I was feeling sooooo very crappy from fibromyalgia or ME/CFS or Long Covid, it made me feel at least a little bit better to know what was causing the crappiness.

So we’re gonna get into the science of a Herx.  If you’re feeling a little lost in the science-ey stuff, take a look at the handy chart we’ve created for you (below all the science-ey stuff).

Keep in mind that this explanation is giving you a simplified version of what happens.

A Herxheimer Reaction basically has three different stages:

  1. Pathogen dies, which includes a few different elements
  2. The immune system responds
  3. The body gets rid of the toxins

Stage 1: Pathogen dies

When we’re treating hidden infections and a pathogen like worms (helminths), yeast, candida, fungi, protozoa, bacteria, borrelia, etc., dies, it starts a chain reaction of several processes in our body that can create new symptoms or symptom flares.

  • First, either the biofilm that holds the pathogen (parasite, fungus, candida, etc)  is broken down, and/or then the pathogen itself is broken down.
  • The pathogens are toxic themselves and additionally hold toxins (and sometimes even other pathogens…for example, worms usually have their own little microcosm of yeast and fungi!) within their cell walls.  Once they are killed/broken down, those toxins and other pathogens are released.  These toxins include things like glycoconjugates, which is a broader term for the types of toxins that yeast, bacteria, fungus, and parasites produce and have within them.  These toxins affect our bodies in various ways.  Some cause brain fog, some muscle weakness, some nervous system issues, some anxiety, some heart palpitations, etc.

So now that you’ve killed the parasites, you’ve got all these toxins and maybe some newly released pathogens floating around…what do you think your amazing, super smart body does next?

Step 2:  The immune system responds

Our bodies are built really really well, so when our immune system detects all of these new toxins and newly released pathogens, it kicks in hardcore to manage with a pro-inflammatory response.  As an example…if you’ve found you’re experiencing higher temperatures/flushing/fever as you’re treating, this could be because of bacteria die-off.  Bacteria release lipopolysaccharides (LPS), which can induce fever (which is ultimately the body trying to take care of itself!).  Similarly, other symptoms, whether new symptoms or symptom flares, are a natural inflammatory response of the immune system and needed in order to overcome the infections and toxins that have been released.

This is why sometimes when my H*E*A*L* Program : Recovering from Long Covid and ME/CFS course. students tell me that their brain fog has gotten worse, or their fatigue, their GI issues,  their pain, or their (fill in the blank)…or they have new symptoms, I am, to their surprise, excited!  I’m excited because it usually happens because the treatment is working.

3. The Body gets rid of toxins – DETOX

Where does all the broken down biofilm and subsequent released toxins and parasite remnants go?  In an optimally functioning body, most of these toxins would move through the large intestine out of the body through fecal matter (poop!).  Unfortunately, most people with chronic illness health conditions like Long Covid, ME/CFS, Fibromyalgia, etc., have compromised GI tracts and have developed a leaky gut.

The simple explanation of a leaky gut is that it allows macromolecules and microorganisms to permeate the intestinal lining and go into the bloodstream.  Once in the bloodstream, your immune system does its job of attacking the toxins using your lymphatic system and liver first, and then through your kidneys, colon, and skin.

The thing is, most people with chronic illness have developed compromised lymphatic systems, liver, kidneys, among other detox systems for a number of reasons.  This means that the body is unable to eliminate the toxins and they end up recirculating in the body and making the inherently nasty symptoms that go along with killing pathogens in the body last even longer and show up in even more pronounced ways.

So this is why it feels so crappy!

Common Triggers of Detoxification Symptoms

Given that we now have some insight into the processes behind detoxification symptoms, let’s look at the most common triggers. Whether they arise from:

  • Cell death
  • The release of toxins
  • Inflammation,
  • Or the body’s challenges in expelling these toxins

The next question becomes: what instigates such alterations? The three primary causes frequently associated with detoxification symptoms are as follows:

Initiating an elimination diet: Adopting a real-food-centric diet that excludes processed foods, sugar, and other elements that nourish pathogens often leads these harmful organisms to starvation, as their food sources are severed. This effect is more pronounced in specific elimination diets for conditions like irritable bowel syndromes (IBS), such as low-FODMAP, Paleo AIP, and SIBO Bi-Phasic, which rigorously eliminate foods supporting pathogenic growth (e.g., sugar and other carbohydrates).

Introduction or increase of probiotics intake:  Starting a probiotic regimen can effectively eliminate pathogens by overpowering them with beneficial microorganisms, often referred to as ‘good guys.’

Implementation of an antimicrobial, anti-fungal, or anti-parasitic supplement protocol: This method is the most common and efficient approach to eradicate pathogens, usually resulting in heightened detoxification symptoms. Consequently, individuals may worry about the potential side effects of substances like Biocidin or oregano, not realizing that they might simply be undergoing detoxification symptoms, indicative of the supplement fulfilling its intended purpose.

Without the appropriate supportive products, people may encounter substantial detoxification symptoms related to conditions such as Small Intestinal Bacterial Overgrowth (SIBO), Helicobacter pylori (H. pylori) infection, and parasite infestations—all linked to common underlying mechanisms within the body.’

Why Not All Symptoms Are Attributable to Die-Off

Before looking into a more comprehensive discussion of common die-off symptoms, it’s crucial to recognize that not every symptom encountered during the treatment of parasites, bacteria, or yeast overgrowths is necessarily linked to die-off.

There is a tendency to categorize various reactions as die-off symptoms, but such generalization may not always be accurate. Here are alternative factors that could contribute to symptom exacerbations during elimination:

Reactions to supplements or food. patients with gut dysfunction and a leaky gut often experience common food intolerances and sensitivities. Symptom flares may be attributed to immune responses against specific food proteins, such as those found in:

  • Eggs
  • Nuts
  • Gluten
  • And dairy

Rather than the death of pathogens.

Parasite life cycles. Parasites typically undergo life cycles ranging from 7 to 21 days, leading to potential symptom flares during specific phases. Consistent patterns of feeling well for a week or two followed by a period of feeling unwell may indicate the parasites are still active rather than dying off.

Alternate sources of inflammation. As inflammation is a central aspect of die-off symptoms, it’s easy to confuse them with other sources of inflammation. These alternative inflammatory triggers may include mental or emotional stress, new gut infections, excessive exercise, or other forms of stress on the body.

Symptoms of Bacterial, Yeast, and Parasite Die-Off

Die-off symptoms often closely mirror the symptoms associated with infections caused by pathogens, as they arise from the same types of toxins. The key distinction lies in the fact that die-off symptoms related to bacteria, yeast, and parasites tend to be more pronounced, given the larger quantities of toxins released within a shorter timeframe. It is common to experience a temporary worsening of symptoms before improvement occurs. This holds true for individuals undergoing protocols to address conditions such as:

The most prevalent symptoms associated with die-off from bacteria, yeast, and parasites include:

  • Fatigue
  • Headaches
  • Body aches and pains
  • Skin rashes or reactions
  • Mood symptoms (e.g., anxiety, depression, irritability)
  • Respiratory symptoms such as phlegm or a stuffy nose
  • Increased gastrointestinal upset
  • Flu-like symptoms (e.g., fever and chills

Fatigue

Fatigue stands out as one of the most frequently reported die-off symptoms among clients. The process of healing can be demanding, especially when the body is actively combatting a significant burden of pathogenic infections or parasites. In the initial stages of an elimination protocol, it is essential to allow ample rest, enabling the body to focus on the healing process without introducing additional stressors.

Headaches

Detox and headaches often coincide for many clients. While the cell death of pathogens can contribute to this symptom, for many, the primary factor behind headaches is the elimination of certain inflammatory foods, such as caffeine and sugar, from their diet.

Body Aches and Pains

Body aches and pains manifest as highly inflammatory symptoms. During die-off, the immune system’s inflammatory response to the release of toxins can result in muscle soreness and joint aches if the response is overly active.

Skin Rashes and Reactions

The skin, a crucial elimination organ, may attempt to expel excess toxins if other bodily systems are overwhelmed. Individuals with pre-existing skin issues may witness a temporary increase or exacerbation of,

  • Rashes
  • Eczema
  • Psoriasis flares
  • Skin sores
  • Or breakouts.

While these reactions are typically short-term, managing them can be challenging.

Mood Symptoms (e.g., Anxiety and Depression)

The well-established link between gut health and mood symptoms is evident, and mood-related issues like anxiety and depression can intensify during pathogen elimination. Acknowledging that these mood symptoms are a consequence of temporary pathogen die-off rather than external life circumstances can make this phase more bearable.

Respiratory Phlegm or Stuffy Nose

Mucus and phlegm play a role in capturing and eliminating toxins from the body, similar to the response observed in colds or flu. An increase in mucus production is a common die-off sign as the body mobilizes various systems to excrete surplus toxins.

Increased GI Upset

The active transformations in the gut during die-off often lead to heightened gastrointestinal upset. Diarrhea, constipation, bloating, gas, and abdominal pain are frequently reported as pathogens undergo cell death and release toxins into the gut. GI symptoms are also prone to be associated with non-die-off causes, such as food reactions and general symptom flares related to parasite life cycles.

Flu-Like Symptoms (e.g., Fever and Chills)

Flu-like symptoms, originally identified as the Herx reaction a century ago, represent a more severe form of die-off symptomatology. This type of reaction is typically linked to spirochetal infections treated with potent antibiotics. Fever, a mechanism the body employs to combat infections, is less common in natural antimicrobial herb treatments due to lower doses, extended timeframes, and titration practices.

Managing Symptoms of Die-Off

Functionally addressing die-off symptoms requires a comprehensive understanding of the underlying mechanisms causing them. In the initial part of this post, we looked into the reasons behind die-off symptoms, highlighting three critical factors influencing their severity:

  1. Quantity and speed of pathogen cell death – determining the magnitude of the ‘toxic burden’ the body must handle.
  2. Activity level of the immune system – influencing the extent of inflammation generated by the body.
  3. Efficiency of detox pathways – impacting the body’s ability to clear toxins.

In light of these factors, our approach to minimizing die-off symptoms encompasses three interconnected elements, which we will discuss further:

  1. Titration – Gradual introduction of dietary changes and supplements.
  2. Reduction of inflammation – Alleviating the body’s immune response.
  3. Support for detox pathways – Assisting the body in toxin elimination through detox organs.

Titration of Diet and Natural Supplements

Titration involves the gradual introduction of specific interventions. The speed and intensity of an intervention correlate with the rapid release of toxins from dying pathogens. The goal is to avoid a scenario where an overgrown weed garden is metaphorically bombed, leading to the dispersal of spores throughout the neighborhood. Instead, we opt for a more selective weeding approach to mitigate negative repercussions.

To prevent or minimize die-off symptoms for our clients, we employ three titration strategies:

  • Natural antimicrobials – Unlike antibiotics, natural antimicrobials or herbal supplements offer a gentler approach to addressing pathogens and parasites. While they can still induce die-off symptoms, these are generally less severe than those caused by antibiotics.
  • Diet introduction – We initiate clients on a gut-healing diet after the initial consultation, allowing for a month of diet adjustments to settle before commencing a supplement protocol and introducing antimicrobials.
  • Supplement titration – We introduce one supplement at a time, gradually building up each product to full dosage before moving to the next. This approach allows us to monitor reactions separately, adjust dosing recommendations, and facilitate a slow build-up to the pathogen eradication process, reducing the quantity and speed of cell death that the body must manage. This contrasts with conventional antibiotic recommendations that start at full dose from day one, often resulting in more significant die-off symptoms.

Reducing Inflammation

High levels of inflammation can lead to various die-off symptoms, including fatigue and body aches. We manage inflammation through two primary avenues:

  • Rest and stress reduction – Providing the body with the space to fight and heal from pathogenic infections involves prioritizing quality sleep, particularly between 10 pm and 2 am when significant physical repair occurs. Avoiding stress-inducing activities, especially intense or excessive exercise, is crucial to overcoming inflammation.
  • Anti-inflammatory foods and supplements – Supporting the body with natural anti-inflammatory elements can alleviate the severity of inflammation-related die-off symptoms. Key anti-inflammatory supports include:
  • Omega-3s from quality fish oil or cod liver oil supplements, as well as food sources like flaxseed, chia seeds, walnuts, and oily fish.
  • Spices like turmeric, ginger, and cinnamon, added to food or consumed in tea.
  • Antioxidants from green leafy vegetables and dark-colored berries.
  • Additional supplements such as glutathione, quercetin, and vitamin C can be beneficial when needed.

Supporting Detoxification Pathways

Facilitating the clearance of excess toxins through our detox organs is a crucial aspect of both preventing and mitigating the severity of die-off symptoms. We’ve identified two highly effective methods to support these detox pathways:

Liver Support: The liver takes center stage as the primary organ responsible for removing bacteria, their lipopolysaccharide (LPS), and other glycoconjugates from parasites and yeast. This detoxification function significantly contributes to the reduction of die-off symptoms. Our approach involves bolstering both phase 1 and phase 2 liver detoxification pathways, typically utilizing supplements that combine natural ingredients like:

Lab testing markers help assess phase 2 liver detoxification, guiding the need for additional support during the protocol. A preferred product for liver support, with added gastrointestinal detox support, [{Let me know which product to add here}is Pure Encapsulations, Liver-G.I. Detox.]

GI Binders: GI binders act like magnets, binding toxins and transporting them through the digestive tract for elimination in the stool. By effectively clearing excess toxins, GI binders prevent their absorption (or reabsorption) into the bloodstream, thereby reducing further die-off symptoms. We commonly use three types of binders to help clients minimize die-off symptoms:

  • Charcoal: Activated charcoal, a broad-spectrum binder, is highly effective in acute cases of die-off symptoms. While it binds to microbes, minerals, vitamins, and nutrients, it is best used for short-term relief, taken well away from other foods, medications, or supplements. It serves as an affordable and quick solution to address die-off symptoms promptly.
  • Clays: Bentonite, zeolite, and pyrophyllite clays are more selective than charcoal, binding to some nutrients. This makes them a gentler form of binder suitable for extended use over weeks instead of days.
  • Chlorella: As an algae with an affinity for toxins, chlorella serves as an ongoing support product for die-off and toxins associated with eradication protocols. It binds to toxins without affecting essential nutrients, making it a suitable ongoing preventative support throughout the protocol. While it may not provide immediate relief once die-off symptoms manifest, charcoal and clays can address that aspect.
  • Binder Blends: To offer our clients the benefits of various binders in a single supplement, we often recommend blends like Biocidin Botanicals, G.I. Detox +.

Additional Strategies for Detox Support and Minimizing Die-Off Symptoms

In addition to the aforementioned approaches, we frequently recommend the following strategies to enhance our clients’ detox pathways and alleviate symptoms of die-off:

  • Hydration – A straightforward yet impactful tactic, maintaining adequate fluid intake throughout a protocol can assist in flushing toxins from the system.
  • Epsom Salt Baths – Epsom salt baths offer various benefits, including relaxation, potential elevation of magnesium levels to address aches and pains, toxin removal from the skin, and an increase in sulfate levels beneficial for liver detox.
  • Infrared Sauna – Some clients have reported positive effects from using infrared saunas during a protocol. Whether attributed to their relaxing nature or their purported ability to boost blood circulation and stimulate sweat glands for releasing accumulated toxins, results may vary among individuals. It’s crucial to stay hydrated and replenish electrolytes if opting to explore infrared saunas

Effectively managing die-off symptoms during pathogen eradication involves a holistic approach that considers the quantity and speed of pathogen cell death, the immune system’s activity, and the efficiency of detox pathways. Employing titration methods, reducing inflammation through rest and anti-inflammatory supports, and supporting detox organs play pivotal roles in minimizing the impact of die-off symptoms.
